  • Full Name
  • Hair Color
  • Eye Color
  • Sex
  • Race
  • Date of Birth
  • Place of Birth
  • Height
  • Weight
  • Country of Citizenship
  • Social Security Number
• Make sure everything is legible and clear
• Fingerprints cannot be smudged
• Your EFT will be emailed to you
• Your FD258 fingerprint card will not be returned, it will be shredded after 10 days of order fulfillment
